Investigation Request / Complaint Form

Spring will be here soon, and so will the noxious weeds!  If you choose to fill out an Investigation Request form, it will be logged into our system and looked at when our Seasonal Field Crew returns in April.

Inspections begin with a written complaint:  form submitted online, mail or fax.
