Below you will find the information and supporting forms for filing and reporting for Trusts.
Completed forms are required for court hearings and reporting. Incomplete forms will cause delays and may result in the need to reschedule your hearing. Please be sure completed documents conform to all state and local rules governing acceptable document format. The court will only accept documents that are “one sided.” Do not file documents with printing on both sides of the paper.
Forms are available in Word and PDF. These forms are not to be modified from the version available. Please type forms or use legible handwriting in blue or black ink, and allow a one-inch margin plus a three-inch header on the first page for each form. No forms (including bench copies or financial audit responses) are accepted via fax or email.
